What can support coordination be used for?
Support coordination funding helps a participant put their plan into action. That includes understanding the plan, connecting with disability, mainstream and community supports, and building the skills and confidence to coordinate their own supports over time. There are three levels, and it is different from plan management and direct support delivery.

The three levels
Support coordination is delivered at three levels. Support connection helps a participant use their plan and build links to supports. Coordination of supports helps them design and manage a mix of supports and resolve issues. Specialist support coordination applies where a participant's situation is more complex and needs a specialist practitioner. The right level depends on what is funded in the plan.
What it can be used for
Typical activities include explaining the plan, finding and connecting with providers, setting up service agreements, coordinating a mix of formal and informal supports, and helping a participant prepare for a plan review. A core purpose is capacity building, so the participant becomes more able to manage their own supports.
What it is not for
Support coordination does not cover the hands-on delivery of other supports, such as personal care or therapy, and it is not the same as plan management, which handles the financial side of the plan. It should not be used for tasks unrelated to the participant's disability or plan goals. Keeping the scope clear helps the funding last and stands up at plan review.
